We are keen to encourage patients to choose the type of hearing they have – in person, video or telephone conference – and, where possible to hold that type of hearing. Requests for Alteration of Hearing Date, Time or Place (civil and CORO cases)

The above form should be used for requests to change the date of a hearing under rule 57. Requests to adjourn which are made close to the date of a hearing should be sent with an explanation of the reason for seeking adjournment, and the position of other attendees if at all possible.

Compulsory Treatment Order (CTO) and Compulsion Order (CO): Applications; Extensions; Variations etc

Please note that the following forms have been revised: CTO1, CTO2, CTO3a, CTO4, CTO5, CTO10 and CO1 and that only these new versions of the forms should be used in future.
